1. Accuracy and good-faith reporting I confirm that the information I am submitting is accurate and complete to the best of my knowledge and belief. I have identified which information is based on my personal observations and which information I received from someone else, and I have not knowingly included false, misleading, or fabricated information. I understand that Bentley's Playhouse may decline to act on a tip, may seek to verify information, and may refer it to an appropriate agency. I understand that knowingly providing false information to law enforcement may have legal consequences under applicable law. (required)
2. Photos, videos, documents, and other materials I confirm that I created the photos, videos, documents, or other materials I am submitting, or that I have the legal right and permission to provide them for the purposes described in this agreement. I will not knowingly submit material that violates another person's copyright, privacy, confidentiality, or other legal rights. I understand that submitting material does not transfer its ownership to Bentley's Playhouse. (required)
3. Permission to review and share the tip I grant Bentley's Playhouse a nonexclusive, royalty-free license to receive, store, review, reproduce, and share the information and materials I submit as reasonably necessary to assess or verify the tip, seek assistance for an animal, make a referral, communicate with relevant animal services agencies, law enforcement, veterinarians, animal-welfare organizations, or legal advisers, and maintain records of the report. This permission does not authorize public posting or publication of my submission or identifying information unless I separately agree or disclosure is required or permitted by law. I understand that Bentley's Playhouse does not guarantee that it will investigate, refer, publish, or take any particular action on a submission. (required)
4. Name, credit, or anonymous submission I understand that Bentley's Playhouse will make reasonable efforts to follow the name, credit, or anonymity preference I selected on this form. I understand that an anonymous submission may still contain details that could identify me, and that Bentley's Playhouse cannot guarantee confidentiality or prevent identification by others. If I later change my preference, the change will apply prospectively after Bentley's Playhouse receives it and has a reasonable opportunity to act. It may not be possible to retrieve or change materials already shared with another person or organization, or content already published with my prior permission. (required)
5. Lawful collection and submission of information I confirm that I have not knowingly obtained the information or materials I am submitting through trespassing, harassment, unauthorized access to an account or device, or other unlawful conduct. I will not put myself or another person at risk to gather or submit information. I understand that Bentley's Playhouse does not encourage or authorize unlawful conduct, and that submitting information does not give me permission to enter private property or confront anyone. 6. No emergency response or guaranteed outcome I understand that Bentley's Playhouse is not an emergency service, law-enforcement agency, animal-control authority, veterinarian, or legal representative. Submitting this form does not ensure that anyone will respond within a particular time or that an animal will be located, removed, treated, rescued, or placed. If an animal appears to be in immediate danger, I should contact 911 or the appropriate local animal-control or law-enforcement agency directly. (required)
7. No confidential or professional relationship I understand that submitting a tip does not create an attorney-client, veterinarian-client, agency, fiduciary, or other professional relationship with Bentley's Playhouse. I should not assume that my submission is confidential or legally privileged. I will avoid including unnecessary sensitive personal information about myself or others. (required)
8. Responsibility for a false or unauthorized submission To the extent permitted by law, I agree to be responsible for claims and losses arising from my knowing misrepresentation that I had the right to submit information or materials, my knowing submission of false information, or my breach of the promises I made in this agreement. This provision does not waive any rights or remedies that cannot legally be waived. (required)
