Foster Application

Thank you for wanting to foster a dog with Bentley's Playhouse Animal Rescue and Sanctuary. Fosters give our dogs a safe place to heal and learn while they wait for their family. Please answer as completely as you can, so we can match the right dog with the right home.

Fosters must be 18 or older. Your answers are saved in this browser as you go, so you can come back and finish later.

1About You

If you're under 25, we'll need written consent from a parent or legal guardian.

Please also email a photo of your license to hello@bentleysplayhouse.org.

2References

Please fill these out completely.

3Your Household

Write "None" if no children live with you.

4Your Home

Please email photos of your home to hello@bentleysplayhouse.org: shared living areas, bedrooms, the kitchen, anywhere the dog will spend time, and the front, side and back yard if you have one. If you rent, we'll need written confirmation from your landlord that pets are allowed and what the weight limits are.

Size, location, yard size, stairs, which floor you live on, whether there is an elevator and so on.

Do you own or rent? (required)
What type of home do you live in? (required)
Will the dog have to use stairs at any point, for example to reach your apartment or a second floor? (required)
Does the property have a fenced yard? (required)

5Experience With Pets

Having owned a pet or not doesn't make you more or less qualified to foster. This just helps us understand your experience.

Dog training, vet tech, groomer, agility, happy pet owner and so on.

Write "None" if you don't.

If you have a dog, is it friendly with other dogs? (required)

Write "No" if you aren't.

Are those fosters spayed or neutered? (required)

6Caring for Your Foster Dog

Do you have cameras at home to check on the dog, or would you be open to them? (required)
How often do you travel? Are you planning a vacation in the next one to two months? (required)
Have you ever crate trained a dog? (required)
Are you willing to have someone from Bentley's Playhouse visit your home? (required)
Are you willing to cover small incidental costs for a foster dog? (required)

Bentley's Playhouse provides all supplies and pays for regular vet visits. Incidentals are small things the dog might need before we can get them to you, like a collar or leash, training pads, a bed or food.

7Dog Fighting

Are you aware that dog fighting exists, and that people look for "bait dogs"? (required)
Do you agree that dog fighting is animal abuse, that it should end and that those involved should be prosecuted to the fullest extent of the law? (required)
Are you involved in any business related to animal fighting or trafficking? (required)

8Foster Care Agreement

Please type your initials next to each statement. Your electronic signature is legally binding.

I understand and agree with everything explained to me during the application. If my foster dog is on medication, I will give it exactly as directed. I will never let the dog out loose on its own. If the dog needs medical care, I will contact Bentley's Playhouse first unless it's an emergency. If the dog needs medical care because of my negligence, I will pay for all of the treatment. The foster dog belongs to Bentley's Playhouse, and I will not sell, trade or give it away. My own pets and any other fosters are up to date on their standard vaccinations, and I have given Bentley's Playhouse a current shot record for each one.

Anyone who wants to adopt my foster dog, including me, must go through the standard adoption process. Every adoption or placement decision is up to Bentley's Playhouse alone. My recommendations as the foster are taken into account.

This is an application to volunteer as a foster. Bentley's Playhouse has made no promises about whether I'm eligible to foster certain animals.

I will not bring other dogs or cats into my home while fostering, because that could expose the dog to disease or an attack and put us both in an unsafe situation.

Fostering is a volunteer role, not a paid job, and I will not be paid by Bentley's Playhouse or its partners. Any donations made for the dog, to me or to the rescue, go to the dog's care. If I raise money myself, I do it by my own choice, I am responsible for it, and I will use it only for the dog's care and never for myself. Bentley's Playhouse is a nonprofit and does not raise money for personal use.

This agreement is only for fostering. The dog stays the property of Bentley's Playhouse.
